• To request health care records, sign into the patient portal and fill out a record request form

  • The requested forms will be made available to you through the patient portal within 5 business days from receipt of the completed form

• To contact the Texas Behavioral Health Council, go to the Council's Contact Us webpage

• To file a consumer complaint, go to the Office of Attorney General’s Consumer Protection webpage
